ABSTRACT

Background: Sciatic nerve is a mixed nerve which provide sensory and motor supply for skin and muscles of the lower limb by tibial and common peroneal nerve. Anatomical variations of sciatic nerve at high division have been reported by various authors. The path of the sciatic nerve is important while administration of intramuscular injection to prevent nerve injury and nerve blockage failure during anesthesia. This knowledge of high division helps in different surgical approach for sciatic nerve injury or hip dislocation. Aims and Objectives: The aim of the study was to describe incidences of high division variation of Sciatic nerve and different types in cadavers during routine dissection schedule. Materials and Methods: The study was conducted during routine dissection schedule in anatomy department for first MBBS students to observe sciatic nerve course in 30 gluteal regions from 15 adult cadavers fixed by formalin. Location of sciatic nerve in relation to piriformis muscle and its division whether in single nerve sheath or separate sheath and types was recorded. Results: In 12 cadavers (80%), sciatic nerve course found normal which leaves pelvis at inferior border of piriformis muscle and bifurcate in terminal branches tibial nerve and common peroneal nerve as it approaches at the apex of popliteal fossa. In 3 cadavers (20%), two male and one female, we found high division of sciatic nerve where terminal branches, tibial nerve, and common peroneal nerve leave the pelvis below piriformis separately in different sheaths. Conclusion: Knowledge of variations-related high division of sciatic nerve would help surgeons during different interventions related to sciatic nerve and for preventing further complications.

OBJECTIVE@#To observe the analgesic effect of Tuina by pressing and kneading the Huantiao (GB30) acupoint on rats with chronic constriction injury (CCI) and to explore the analgesic mechanism of Tuina on sciatica rats.@*METHODS@#Thirty-two SPF male SD rats weighing 180 to 220 g were randomly divided into fore groups:blank group (without any treatment), sham group (only exposed without sciatic nerve ligating), model group (sciatic nerve ligating) and Tuina group (manual intervention after lsciatic nerve ligating). The CCI model was prepared by ligating the right sciatic nerve of the rats, on the third day of modeling, the rats in the Tuina group were given pressing and kneading the Huantiao (GB30) point for 14 days, and the changes of paw withdrawal threshold(PWT), paw withdrawal latency(PWL) were measured before and on the 1st, 3rd, 7th, 10th, 14th and 17th days after modeling. The changes of sciatic functional index(SFI) were measured before and on the 1st and 17th day after modeling. The morphological changes of the sciatic nerve were observed by hematoxylin-eosin(HE) staining;and the differences in NF-κB protein expression in the right dorsal horn of the spinal cord of rats were detected.@*RESULTS@#Following modeling, there was no significant difference in PWT, PWL and SFI between the blank group and the sham group (P>0.05), but the PWT, PWL and SFI of the model group and the Tuina group decreased significantly (P<0.01). After manual intervention, the pain threshold of rats in Tuina group increased. On the 8th day of manual intervention (the 10th day after modeling), PWT in Tuina group increased significantly compared with that in model group (P<0.01). On the 5th day of manual intervention (the 7th day after modeling), the PWL of the massage group was significantly higher than that of the model group (P<0.01). The pain threshold of rats in Tuina group continued to rise with the continuous manipulation intervention. After 14 days of manipulative intervention, the sciatic nerve function index of rats in the Tuina group increased significantly(P<0.01). Compared with the blank group and sham group, the myelinated nerve fibers of sciatic nerve in the model group were disordered and the density of axons and myelin sheath was uneven. Compared with the model group, the nerve fibers of rats in the Tuina group were gradually continuous and the axons and myelin sheath were more uniform than those in the model group. Compared with the blank group and sham group, the expression of NF-κB protein in the right spinal dorsal horn of the model group was significantly increased(P<0.01). Compared with the model group, the expression of NF-κB protein in the right spinal dorsal horn of rats in Tuina group decreased significantly(P<0.01).@*CONCLUSION@#Pressing and kneading the Huantiao (GB30) point restores nerve fiber alignment;and improves the PWT、PWL and SFI in the CCI model by decreasing NF-κB p65 protein expression in the spinal dorsal horn. There fore, Tuina demmstrates an analgesic effect and improves the gait of rats with sciatica.

ABSTRACT Facet joint ganglia are benign cystic lesions located adjacent to a facet joint. The majority is asymptomatic. However, can cause important low-back pain and radiculopathy. Neurogenic deficit, claudication, and cauda equina syndrome have also been reported. The authors report two cases of acute low back pain with bilateral sciatica, dorsal foot dysesthesia, and hallux dorsiflexion/extension deficit, due to the presence of encapsulated cysts adjacent to the facet joints causing a significant reduction of the spinal canal. Urgent surgical decompression was performed in both patients with an uneventful recovery. Symptomatic facet joint ganglia is a highly unusual cause of back pain, although it can present with acute onset of bilateral sciatica and canal stenosis requiring urgent surgical decompression. This paper highlights facet joint synovial as a differential diagnosis of lumbar pain and describes two different surgical approaches with good outcomes. Level of Evidence IV; Case Series.


RESUMO: Os quistos facetários são lesões císticas benignas localizadas adjacentes a uma articulação facetária. A maioria é assintomática. No entanto, podem ser causa de importante dor lombar e radiculopatia. Estão ainda relatados casos de déficit neurogénico, claudicação e síndrome de cauda equina. Os autores apresentam dois casos de dor lombar aguda com ciatalgia bilateral, disestesia do dorso do pé e défice na dorsiflexão/extensão do hálux, causados por uma redução significativa do canal medular devido à presença de quistos encapsulados adjacentes às articulações facetarias. Os doentes foram submetidos a descompressão cirúrgica urgente com uma excelente recuperação. Os quistos facetários sintomáticas são uma causa rara de lombalgia, porém podem apresentar-se inicialmente com um quadro agudo de ciatalgia bilateral e estenose canalar com necessidade de descompressão cirúrgica urgente. Este artigo realça os quistos facetários como diagnóstico diferencial de lombalgia e descreve duas abordagens cirúrgicas diferentes com bons resultados. Nível de Evidência IV; Série de Casos.


RESUMEN: Los quistes facetarios son lesiones quísticas benignas situadas junto a una articulación facetaria. La mayoría es asintomática. Pero pueden causar dolor lumbar y radiculopatía importantes. También se han descrito déficit neurogénico, claudicación y síndrome de cauda equina. Los autores presentan dos casos de lumbalgia aguda con dolor ciático bilateral, disestesia del dorso del pie y déficit en la dorsiflexión/extensión del hallux, causados por una reducción significativa del canal medular debido a la presencia de quistes encapsulados adyacentes a las articulaciones facetarias. Los pacientes fueron sometidos a descompresión quirúrgica urgente con una excelente recuperación. Los quistes facetarios sintomáticos son una causa poco frecuente de lumbalgia, aunque pueden presentarse inicialmente con un cuadro agudo de dolor ciático bilateral y estenosis del canal que requiere descompresión quirúrgica urgente. Este artículo destaca los quistes facetarios como diagnóstico diferencial de la lumbalgia y describe dos abordajes quirúrgicos diferentes con buenos resultados. Nivel de Evidencia IV; Serie de Casos.

ABSTRACT

Resumo A persistência da artéria isquiática é um remanescente embriológico da artéria ilíaca interna que ocorre em 0,03% a 0,06% da população, podendo evoluir com degeneração aneurismática. A presença do aneurisma pode levar a embolização, com aumento de risco de perda do membro, principalmente se a artéria isquiática for seu principal suprimento arterial. O tratamento do aneurisma de artéria isquiática está indicado sempre que diagnosticado, devido ao alto risco de complicações. Entre as opções de tratamento, estão o tratamento aberto convencional, o tratamento endovascular e o tratamento híbrido. No presente estudo, será descrito o caso de um paciente apresentando persistência completa das artérias isquiáticas bilateralmente, com degeneração aneurismática de ambas, corrigida de forma endovascular com stent recoberto Covera® (Bard Medical, Geórgia, Estados Unidos).


Abstract A persistent sciatic artery is an embryological remnant of the internal iliac artery that occurs in 0.03% to 0.06% of the population and may develop aneurysmal degeneration. Aneurysms can lead to distal embolization with increased risk of limb loss, especially if the sciatic artery is the main arterial supply to the limb. A sciatic artery aneurysm must be treated whenever diagnosed, because of the high risk of complications. Treatment options include open, endovascular, or hybrid repair. This manuscript describes a patient with bilateral persistence of the sciatic arteries, both with aneurysmal degeneration, who underwent endovascular repair with Covera® (Bard Medical, Georgia-USA) covered stents.

ABSTRACT Objective: To evaluate the clinical profile, pain improvement, and the need for surgical interventions in patients undergoing transforaminal block with the use of corticosteroids and anesthetics. Methods: This is a prospective, randomized, double-blind study with 45 patients with unilateral radicular pain in their lower limbs and a single-segment lumbar disc herniation diagnosis. In the intervention group, transforaminal blocks with bupivacaine, dexamethasone, and clonidine were applied and in the control group, distilled water and bupivacaine. The Oswestry questionnaire was applied. Results: We included 24 female (53.4%) and 21 male patients (46.6%). Of those with an occupation, 85.71% (n = 30) were relieved from their duties due to their illness and 14.29% (n = 5) continued to work with limitations. Those who underwent transforaminal block with an injection of corticosteroids, clonidine, and anesthetics showed immediate relief. However, such effect failed to alleviate patients' symptoms after three weeks. We observed that 52% of patients showed varying degrees of improvement. The control group experienced mild pain relief after one week, which also failed to last after three weeks. Moreover, 50% of patients improved in varying degrees. Conclusion: Further studies with larger samples, new epidemiological data, and longer follow-ups are necessary to validate our hypotheses. Level of Evidence II, Prospective Study.


RESUMO Objetivo: Avaliar o perfil clínico, a melhora da dor e a necessidade de intervenções cirúrgicas em pacientes submetidos ao bloqueio transforaminal com uso de corticosteroides e anestésicos. Métodos: Estudo prospectivo, randomizado e duplo-cego realizado com 45 pacientes com dor radicular unilateral em membros inferiores e diagnóstico de hérnia discal lombar em um único segmento. No grupo intervenção, os bloqueios transforaminais foram feitos com bupivacaína, dexametasona e clonidina; no controle, água destilada e bupivacaína. Foi aplicado questionário de Oswestry. Resultados: A amostra foi composta de 24 mulheres (53,4%) e 21 homens (46,6%). Dos pacientes com ocupação, 85,71% (n = 30) estavam afastados de suas funções devido à doença e 14,29% (n = 5) continuavam a trabalhar com limitações. Os que foram submetidos ao bloqueio transforaminal com injeção de corticoide, clonidina e anestésico apresentaram alívio imediato. Após três semanas, contudo, o efeito não perdurou de forma tão satisfatória, e 52% dos pacientes apresentaram melhora em graus variados. No grupo controle, houve discreto alívio álgico após uma semana, que não perdurou de forma satisfatória após três semanas, com 50% dos pacientes evoluindo para melhora em graus variados. Conclusão: Mais estudos com espaço amostral maior, novos dados epidemiológicos e seguimento mais prolongado são necessários para validar as hipóteses aventadas. ABSTRACT Acute radiating low back pain is a frequently occurring clinical condition among the population, and it represents a significant portion of urgent care in public health services. Objective: Consider the clinical characteristics, demographics, as well as the intensity of the pain, discomfort, and dysfunction of patients who show a clinical diagnosis that is compatible with acute radicular pain, new or reoccurring after an asymptomatic period. Methods: Patients that display a clinical diagnosis that is compatible with acute sciatic nerve pain, with the beginning of it starting within three months, without previous history of a similar occurrence, were seen in an orthopedic health clinic from July 2020 to January 2021. Results: A total of 42 patients were seen with a compatible diagnosis, which represents 1.4% of all medical visits. To the best of our knowledge, no studies have considered the clinical and demographic characteristics of patients with acute radicular pain in the Brazilian population. This study has found a mean value on the disfunction index that is greater than what is suggested by the current literature. Conclusion: About 30% of individuals showed functional involvement that was considered crippling, which presented a stronger association with individuals with the presence of motor deficits, intensity of radiating pain, and professional inactivity. RESUMO A lombociatalgia aguda é uma condição clínica bastante frequente na população e representa uma porção expressiva dos atendimentos de urgência nos serviços de saúde pública. Objetivo: Avaliar as características clínicas e demográficas, bem como a intensidade da dor e da disfunção de pacientes com quadro clínico compatível com dor radicular aguda, inédita ou recorrente após período assintomático. Métodos: Consideraram-se pacientes com quadro clínico compatível com dor ciática aguda que tenha se iniciado em até três meses, sem história prévia de episódio semelhante e que foram atendidos em uma unidade de pronto atendimento ortopédico entre julho de 2020 e janeiro de 2021. Resultados: Foram atendidos 42 pacientes com quadro clínico compatível, representando 1,4% do total de atendimentos realizados no serviço. Não há relatos de estudos que buscaram avaliar as características clínicas e demográficas dos pacientes com dor radicular aguda na população brasileira. Neste estudo, contudo, encontrou-se um elevado valor médio no índice de disfunção em comparação com a literatura atual. Conclusão: Cerca de 30% dos indivíduos apresentaram acometimento funcional considerado incapacitante, sendo que a presença de déficits motores, a intensidade de dor irradiada e a inatividade trabalhista foram estatisticamente maiores nesse grupo em relação aos demais. Majority of patients visiting Ayurvedic hospitals are having ailments related to joints especially low back pain and knee joint pain. Low back ache may be due to many reasons ranges from muscular pain to complicated neurological complaints or malignancy. Disc herniation is also a major reason for low back ache in which radiculopathy might be a typical feature. In all systems of medicine many treatments exist to manage low back ache and sciatica. In Ayurveda also different treatment modalities are present ranging from simple internal medications to complex therapies like Basti (therapeutic enema), to effectively tackle such conditions. The Ayurvedic formulation, Rasna guggulu is mentioned in the treatment of Gridrasi (sciatica). To standardize any formulation its properties should be studied repeatedly in detail. Even though this formulation is comparatively a simpler one consisting of only 3 ingredients, its properties and identifying features are not widely studied. In this study pharmacognostical, organoleptic and physico-chemical characteristics of Rasna guggulu were reported. Pharmacognostical study of this drug reveals the presence of epidermal cells, starch grain, lignified stone cells, prismatic crystals, simple fiber, simple trichome, warty trichome, annular vessels and spiral vessels of Rasna along with oil globules of clarified butter. HPTLC shows 8 and 7 spots at 254nm and 366nm respectively which suggests the presence of 7 – 8 active principles in this drug. This study also can be considered to standardize the formulation, Rasna guggulu.

ABSTRACT

Kati Basti is an Ayurvedic treatment approach that includes several acts such as Bahyasnehana (external oleation), and Swedana Chikitsa (fomentation therapy). Katibasti is not mentioned in any of the classical sources. Even though it is not injected into the body, it is nevertheless referred to as Basti because medicated and lubricated substances are applied to certain parts of the body for a set period. In Kati (back) and Adhah shakagata vikaras, Katibasti is beneficial (disorders of the lower part of the body). This type of external Basti is very beneficial for muscle spasms, lower back tightness, and bone tissue strengthening in the back area. Backache is the common term used for trending diseases of today's lifestyle due to the engagement of people in improper postures, one of them is sciatica. Sciatica (Gridhrasi) is a word for low back pain that travels down the inside of the leg, through the hip, to the back of the thigh, and down the inside of the leg. In general, 5-10% of people with low back pain have sciatica, but the lifetime prevalence of low back pain is believed to be between 49 and 70 percent. Modern science's treatment of sciatica is unsatisfactory, involving the use of analgesics for pain alleviation and surgical techniques that are frequently associated with side effects. Gridhrasi or sciatica is treated with a variety of methods described in Ayurveda. Kati Basti is a one-of-a-kind therapeutic technique due to its simple, non-invasive, and effective approach. The present review focus of Kati basti, its scientific understanding, its mode of action along its application in sciatica.

Abstract Objectives To evaluate the correlation between radiologic changes (Pfirrmann and Modic) and radicular pain intensity in patients who underwent transforaminal endoscopic surgery for lumbar disc herniation. Methods Series of cases with 39 patients, 50 intervertebral discs in preoperative evaluation from January 29, 2018 to August 28, 2019 in an endoscopic spine surgery service. Demographic data, surgical indication, operative details and complications were obtained from medical records. The patients were divided into three groups based on the Modic classification (Modic absence, Modic 1 and Modic 2) and into two groups considering the Pfirrmann classification (Pfirrmann IV and Pfirrmann V). Data were processed in IBM SPSS Statistics for Windows, Version 22.0 (IBM Corp., Armonk, NY, USA), with a significance level of p< 0,05. Results There was no difference between genders; age: 50,36 ± 15,05 years old; disease level: L2-L3 1 (2%), L3-L4 2 (4%), L4-L5 9 (18%), L5-S1 8 (16%), L3-L4 + L4-L5 4 (8%), and L4-L5 + L5-S1 26 (52%); location: right foraminal 7 (14%), left foraminal 15 (30%), central 9 (18%) and diffuse 19 (38%); radicular pain: left 25 (50%), right 11 (22%), and bilateral 14 (28%); preoperative visual analogue scale (VAS): 9,5 ± 0,91, postoperative: 2,5 ± 1,79; surgery duration: 100 ± 31,36 minutes; and follow-up: 8,4 ± 6,7 months. Less postoperative sciatica was registered in the Modic 2 versus Modic 1 group (p< 0,05). There was no difference in the postoperative radicular pain between the Pfirrmann groups (IV versus V). Conclusion Although there is no clinical difference between the groups, in advanced stages of disc degeneration, endoscopic transforaminal discectomy proved to be effective in diminishing radicular pain in patients with lumbar disc herniation.


Resumo Objetivos Avaliar a correlação entre as alterações radiológicas (Pfirrmann e Modic) e a intensidade da dor radicular em pacientes submetidos a cirurgia endoscópica transforaminal para hérnia de disco lombar. Métodos Uma sequência de casos com 39 pacientes, 50 discos intervertebrais em avaliação pré-operatória, no período de 29 de janeiro de 2018 a 28 de agosto de 2019, no serviço de cirurgia endoscópica da coluna vertebral. Os dados demográficos, indicação cirúrgica, detalhes operatórios e complicações foram todos obtidos junto aos prontuários clínicos. Os pacientes foram divididos em três grupos, com base na classificação Modic (ausência de Modic, Modic 1 e Modic 2) e em dois grupos, considerando a classificação de Pfirrmann (Pfirrmann IV e Pfirrmann V). Os dados foram processados no software IBM SPSS Statistics for Windows, versão 22.0 (IBM Corp., Armonk, NY, EUA), com nível de significância de p <0,05. Resultados Não houve diferença entre os gêneros; idade: 50,36 ± 15,05 anos; nível da doença: L2-L3 1 (2%), L3-L4 2 (4%), L4-L5 9 (18%), L5-S1 8 (16%), L3-L4 + L4-L5 4 (8%), e L4-L5 + L5-S1 26 (52%); localização: foraminal direito em 7 pacientes (14%), foraminal esquerdo em 15 pacientes (30%), central em 9 pacientes (18%), e difuso em 19 pacientes (38%); dor radicular: esquerda em 25 pacientes (50%), direita em 11 pacientes (22%), e bilateral em 14 pacientes (28%); escala visual analógica (EVA) pré-operatório: 9,5 ± 0,91, pós-operatório: 2,5 ± 1,79; tempo cirúrgico: 100 ± 31,36 minutos; e acompanhamento de 8,4 ± 6,7 meses. Foi registrada menos dor ciática pós-operatória nos grupos Modic 2 versus Modic 1 (p< 0,05). Não houve diferença na dor radicular pós-operatória entre os grupos Pfirrmann (IV versus V). Conclusão Embora não exista diferença clínica entre os grupos, em estágios avançados da degeneração discal, a discectomia transforaminal endoscópica mostrou-se eficaz na redução da dor radicular em pacientes com hérnia de disco lombar.

OBJECTIVE@#To compare the therapeutic effect between Fanzhen Jieci (warming acupuncture plus fast needling) combined with conventional acupuncture and simple conventional acupuncture on discogenic sciatica.@*METHODS@#A total of 76 patients with discogenic sciatica were randomized into a Fanzhen Jieci group and a conventional acupuncture group, 38 cases in each one. Conventional acupuncture was applied at Shenshu (BL 23), Dachangshu (BL 25), L1-L5 Jiaji (EX-B 2) and Huantiao (GB 30) on the affected side, etc. in the conventional acupuncture group. On the basis of the treatment in the conventional acupuncture group, Fanzhen Jieci was applied at L1-L5 Jiaji (EX-B 2) and Huantiao (GB 30) on the affected side in the Fanzhen Jieci group, i.e. warming acupuncture was applied at L1-L5 Jiaji (EX-B 2), and fast needling was applied at Huantiao (GB 30) on the affected side for a depth of 40-60 mm, so as to introduce a sensation of electric shock transmitting to lower limb, and then the needle was immediately withdrawn. The treatment was given once every other day, 3 times a week for 3 weeks in both groups. The visual analogue scale (VAS) score of leg and low back pain, the Oswestry disability index (ODI) score and the 36-item short form health survey (SF-36) score before and after treatment were compared between the two groups.@*RESULTS@#Compared before treatment, the VAS scores of leg and low back pain and the ODI scores after treatment were decreased in both groups (P<0.001), the changes of the VAS scores of leg and low back pain in the Fanzhen Jieci group were larger than those in the conventional acupuncture group (P<0.05). After treatment, except for the role emotional and health transition scores, the various scores of SF-36 were increased compared before treatment in the Fanzhen Jieci group (P<0.01); except for the role physical, role emotional and health transition scores, the various scores of SF-36 were increased compared before treatment in the conventional acupuncture group (P<0.01). After treatment, the physical functioning, role physical, bodily pain, mental health and general health scores of SF-36 in the Fanzhen Jieci group were higher than those in the conventional acupuncture group (P<0.05).@*CONCLUSION@#Fanzhen Jieci combined with conventional acupuncture can effectively relieve the pain and improve the mental state in patients with discogenic sciatica, its therapeutic effect is superior to simple conventional acupuncture.

Introdução: A síndrome do piriforme (SP) é uma causa de dor ciática pouco entendida e conhecida, por isso, subdiagnosticada. Possui múltiplas possibilidades etiológicas e a abordagem terapêutica corrente privilegia o tratamento conservador. Objetivo: Neste estudo retrospectivo são destacados o diagnóstico, a etiologia, o diagnóstico diferencial e analisados os resultados do tratamento instituído. Métodos: 34 casos com seguimento mínimo de seis meses e máximo de 12 meses avaliados por uma escala simplificada de graduação de sintomas. Resultados: O tratamento clínico-conservador obteve excelentes resultados em 23 pacientes (67,6%), bom em nove pacientes (26,4%) e razoáveis (insatisfatórios) em dois pacientes (5,8%). Conclusões: Na dor com característica ciática, contínua ou intermitente e sem evidências de compressão radicular ou herniação discal lombar, deve-se pesquisar a possibilidade de síndrome do piriforme como um diagnóstico eminentemente clínico e de exclusão. O tratamento conservador apresenta resultados satisfatórios na maioria dos casos e a indicação cirúrgica está reservada como último recurso às falhas da terapia conservadora.


Introduction: Piriformis syndrome is a cause of sciatalgy barely understood and frequently unrecognized. It has multiple possible etiologic factors and the treatment of option is largely conservative. Objective: In this retrospective study, the diagnostic signs, the ethiology, the diferential diagnosis and the treatment results are discussed. Methods: 34 patients were followed-up for 6-12 months and evaluated by a simplified symptom rating scale. Results: Excellent results in 23 patients (67,6%), good in 9 patients (26,4%) and fair (unsatisfactory) in 2 patients (5,8%). Conclusions: In patients with sciatic pain without proved rachidian or discal lumbar disease, the possibility of Piriformis Syndrome must be investigated mainly by proper clinical examination and seen as a diagnosis of exclusion. The conservative treatment has satisfactory outcomes in most of cases and surgical procedure is reserved as a last resort in case of failure of the conservative management.

ABSTRACT

OBJECTIVE@#To investigate the short-term clinical effect of lumbar nerve root canal injection under X-ray angiography in the treatment of sciatica.@*METHODS@#The clincal data of 78 patients with sciatica underwent lumbar nerve root canal injection under X-ray angiography from December 2017 to February 2020 was retrospectively analyzed. Including 31 males and 47 females, aged from 22 to 88 years old with a median of 65 years. There were 55 cases of lumbar disc herniation and 23 cases of lumbar spinal stenosis, the course of disease ranged from 1 to 8 weeks with a median of 3 weeks. There were 71 cases of single segment disc herniation or stenosis, including L3,4 of 5 cases, L4,5 of 61 cases, L5S1 of 5 cases, and 7 cases of multisegment herniation or stenosis. The pain visual analogue scale (VAS) was recorded and Macnab was used to evaluate the clinical effect.@*RESULTS@#All patients completed standardized treatment without serious adverse reactions. VAS were (3.21±0.76) scores immediately after treatment, (2.89±0.33) scores 1 hour after treatment, (1.80±0.27) scores 6 hours after treatment, (1.10±0.20) scores 24 hours after treatment, (2.53±0.35) scores 1 week after treatment and (4.27±0.36) scores 1 month after treatment. There were significant differences in VAS between before treatment(7.83±0.56) and each time period after treatment(P<0.05). According to Macnab low back pain evaluation standard, 42 cases were effective, 34 cases were markedly effective and 2 cases were ineffective within 24 hours after treatment, with an effective rate of 97.4%;38 cases were effective, 25 cases were markedly effective, 15 cases were ineffective within one week after treatment, the effective rate was 80.0%;32 cases were effective, 22 cases were markedly effective, 24 cases were ineffective within one month after treatment, the effective rate was 69.2%.@*CONCLUSION@#The short-term clinical effect of nerve root canal injection under X-ray radiography in the treatment of sciatica is good and it is an effective method to relieve sciatica.

ABSTRACT

SUMMARY OBJECTIVE: The objective of this study is to compare the clinical outcome among patients who are surgically treated for lumbar disc herniation by transforaminal and interlaminar endoscopy techniques. METHODS: For the treatment of lumbar disc herniation, 31 patients were assigned to undergo the interlaminar technique and 24 patients the transforaminal technique. They were evaluated using visual analog scale and Oswestry disability index in the preoperative period, in the first postoperative period, and in the 12th month after the procedure. The clinical results between the two techniques were then compared. RESULTS: Overall, 89.1% of the patients obtained good results, with 12.5% complications in the transforaminal technique and 9.6% in the interlaminar technique. CONCLUSION: Although both the endoscopic techniques, compared in this study, are safe and effective for the surgical treatment of lumbar herniated disc, the interlaminar technique presented significantly better results and lower rates of complications than the transforaminal technique.

ABSTRACT

Although the etiology of sciatica remains uncertain, there is increasing evidence that the disease process of sciatica is associated with the levels of inflammatory factors. Piperine, an alkaloid isolated from Piper nigrum, has previously been demonstrated to inhibit inflammation and analgesic effects. The purpose of this study is to verify the regulatory relationship between miR-520a and p65 and to explore how miR-520a/P65 affects the level of cytokines under the action of piperine, so as to play a therapeutic role in sciatica. Through ELISA experiment, we confirmed that four inflammatory factors (IL-1β, TNF-α, IL-10, TGF-β1) can be used as evaluation indexes of sciatica. The differentially expressed miRNA was screened as miR-520a, by microarray technology, and the downstream target of miR-520a was P65 by bioinformatics. Real-time fluorescence quantitative PCR confirmed that the expression of miR-520a was negatively correlated with pro-inflammatory cytokines, positively correlated with anti-inflammatory cytokines and negatively correlated with p65 expression at mRNA level. The expression of p65 was positively correlated with pro-inflammatory cytokines and negatively correlated with anti-inflammatory cytokines at the protein level verified by ELISA and Western blot. HE staining analysis showed that the nerve fibers were repaired by piprine, the vacuoles were significantly reduced, and the degree of nerve fiber damage was also improved. Immunohistochemical analysis showed that the expression of p65 decreased after administration of piperine. Dual-luciferase reporter gene assay confirmed that the luciferase signal decreased significantly after cotransfection of miR-520a mimics and p65 3'UTR recombinant plasmid. To sum up, in the rat model of non-compressed lumbar disc herniation, piperine plays a significant role in analgesia. MiR-520a can specifically and directly target P65, and piperine can promote the expression of miR-520a, then inhibit the expression of p65, down-regulate the pro-inflammatory factors IL-1β and TNF-α, and up-regulate the effects of anti-inflammatory factors IL-10 and TGF-β1, so as to treat sciatica.

OBJECTIVE@#To compared the clinical effect between triple needling combined with thermo-electroacupuncture at Huantiao (GB 30) and triple needling combined with electroacupuncture at Huantiao (GB 30) for trunk-sciatica.@*METHODS@#A total of 56 patients with trunk-sciatica were randomized into a triple needling combined with thermo-electroacupuncture group and a triple needling combined with electroacupuncture group, 28 cases in each group. In the triple needling combined with thermo-electroacupuncture group, triple needling combined with thermo-electroacupuncture at Huantiao (GB 30) was applied. In the triple needling combined with electroacupuncture group, triple needling combined with electroacupuncture at Huantiao (GB 30) was applied. Both groups were treated with acupuncture at Weizhong (BL 40), Yanglingquan (GB 34) and Chengshan (BL 57). The treatment was given once every 2 days, 10 days as one course for 2 courses. Before treatment, 1, 2 courses into treatment, the scores of pain rating index (PRI), pain visual analogue scale (VAS) and present pain intensity (PPI) were observed, and the clinical efficacy was evaluated in the two groups.@*RESULTS@#The PRI, VAS and PPI scores 1, 2 courses into treatment were decreased in the two groups (@*CONCLUSION@#Triple needling combined with thermo-electroacupuncture at Huantiao (GB 30) could effectively relieve pain in patients with trunk-sciatica, and the curative effect maybe better than triple needling combined with electroacupuncture at Huantiao (GB 30).

OBJECTIVE@#To observe the effect of moxibustion at "Huantiao" (GB 30) on the expression of growth-associated protein-43 (GAP-43) in the sciatic nerve trunk and ventral horn of spinal cord (L@*METHODS@#A total of 48 healthy male SD rats were randomly divided into a normal group, a sham operation group, a model group and a moxibustion group, 12 rats in each group. The rat model of primary sciatic pain was established by chronic constriction injury (CCI) of the sciatic nerve in the model group and the moxibustion group. On the 8th day of the experiment, moxibustion was adopted at "Huantiao" (GB 30) in the moxibustion group for 5-10 min, once a day for 14 consecutive days. Sciatic nerve function index (SFI) was measured and compared in each group at day 1, 7, 14 and 21. On the 21st day of the experiment, HE staining was used to observe the morphology of ventral horn of rat spinal cord and sciatic nerve trunk. Immunohistochemical method and real-time PCR were used to detect mRNA and protein expressions of GAP-43 in the spinal cord and sciatic nerve trunk of rats.@*RESULTS@#On day 7, 14 and 21, there was no statistical difference in SFI between the sham operation group and the normal group (@*CONCLUSION@#Moxibustion at "Huantiao" (GB 30) could improve the sciatic nerve function in rats with primary sciatica and its mechanism may be related to improving the expression of GAP-43 and enhancing the self-repair ability of the sciatic nerve after injury.

ABSTRACT

Los cuadros de lumbociática secundarios a hernia del disco intervertebral (hdi) son frecuentes y causantes de importante morbilidad. En esta revisión narrativa, nos enfocaremos en los aspectos clínicos y de manejo de esta frecuente patología. Se tiene que mantener un algo grado de sospecha, pues no siempre los cuadros clínicos son similares a lo descrito clásicamente. Los signos radiculares irritativos y un examen neurológico exhaustivo son fundamentales, asimismo la correlación entre este cuadro y las imágenes. El curso natural de la hdi es en general hacia la resolución y, por lo tanto, el tratamiento de elección es inicialmente conservador. La cirugía tiene un rol generalmente en pacientes que han fracasado con el manejo conservador. Esta tiene mejores resultados en el corto plazo que el tratamiento conservador continuo, pero en el largo plazo son equivalentes. Es fundamental discutir estos aspectos con el paciente, para lograr una elección informada del tratamiento, de acuerdo a sus preferencias


Sciatic pain caused by a herniated disk (ldh) is frequent and cause of significant morbidity. In this narrative review, we will discuss the clinical aspects and management of ldh diagnosis and management. High suspicion for ldh diagnosis must be kept, since its clinical picture is not always classical. Tension signs and neurological examination are key, along with correlation with images. Ldh natural history is generally towards resolution and therefore, conservative treatment is the first choice. Surgery has its role for patients who have failed conservative treatment. Surgery achieves a faster pain alleviation than conservative treatment, but on the long-term results tend to become equivalent. Ample and detailed discussion of these aspects with the patient are clue for an informed consent and satisfactory results.

ABSTRACT Objective To evaluate whether vertebral endplate signal changes (VESCs) influence the prognosis of patients submitted to conservative or surgical treatment for low back pain and lumbosciatica. Methods Study with 241 patients who underwent conservative treatment, infiltration or surgery with 12 months of follow-up. They were evaluated for pain by the Visual Analog Scale for Pain (VAS), for function by the Roland Morris questionnaire and for quality of life by the EuroQoI5 (EQ-5D). Results The VESCs did not have a significant effect on the treatment responses for the VAS (F = 0.03; P = 0.97), Roland Morris (F = 0.51; P = 0.60) and EQ-5D (F = 2.67; P = 0.07) variables, nor was there any interaction between VESC and treatment for VAS (F = 2.15; P = 0.08), Roland Morris (F = 1.55; P = 0.19) and EQ-5D (F = 2.15; P = 0.08). There was a significant effect for all treatments, however, the effect of the surgical procedure was superior when compared to the others (P <0.001). The VESC frequency was 48.33% for type 0, 29.17% for type I and 22.50% for type II. Conclusions The presence of VESC and its different types is not associated with a worse prognosis, nor was a higher prevalence of VESC observed in the patients with low back pain and lumbosciatica. RESUMO Objetivo Avaliar se as alterações de sinal do platô vertebral (ASPV) influenciam o prognóstico de pacientes submetidos ao tratamento conservador ou cirúrgico em lombalgia e lombociatalgia. Métodos Estudo com 241 pacientes submetidos ao tratamento conservador, infiltração ou cirurgia, com acompanhamento de 12 meses. Foram avaliados pela Escala Visual Analógica (EVA) da Dor, quanto à função, pelo questionário Roland Morris e quanto à qualidade de vida, pelo questionário EuroQoI5 (EQ-5D). Resultados As ASPV não tiveram efeito significante nas respostas do tratamento para as variáveis EVA (F = 0,03; P = 0,97), Roland Morris (F = 0,51; P = 0,60) e EQ-5D (F = 2,67; P = 0,07), bem como não houve interação de ASPV e tratamento para EVA (F = 2,15; P = 0,08), Roland Morris (F = 1,55; P = 0,19) e EQ-5D (F = 2,15; P = 0,08). Houve efeito significante para todos os tratamentos; entretanto, o efeito do procedimento cirúrgico foi superior quando comparado aos demais (P < 0,001). A frequência de ASPV tipo 0 foi 48,33%, tipo I foi 29,17% e tipo II foi 22,50%. Conclusões A presença de ASPV e seus diferentes tipos não estão associados a prognóstico pior, bem como não se demonstrou maior prevalência de ASPV nos pacientes com lombalgia e lombociatalgia. Métodos Estudio con 241 pacientes sometidos al tratamiento conservador, infiltración o cirugía, con acompañamiento de 12 meses. Se evaluaron a través de la Escala Visual Analógica del Dolor (EVA), cuanto a la función, por el cuestionario Roland Morris y cuanto a la calidad de vida por el cuestionario EuroQoI5 (EQ-5D). Resultados Las ASMV no tuvieron efecto significativo en las respuestas del tratamiento para las variables EVA (F = 0,03; P = 0,97), Roland Morris (F = 0,51; P = 0,60) y EQ-5D (F = 2,67; P = 0,07), así como no hubo interacción de ASMV y tratamiento para EVA (F = 2,15; P = 0,08), Roland Morris (F = 1,55; P = 0,19) y EQ-5D (F = 2,15; P = 0,08). Hubo efecto significativo para todos los procedimientos, entretanto, el efecto del procedimiento quirúrgico fue superior cuando comparado a los demás (P <0,001). In present era, attentively engaged in professional and social life, eventually forgotten normal sit and lie postures. Hustles like Jerky movements during travelling, poor postures, bending and lifting awkward etc., creates extreme pressure on the spine which affects functional ability of lower limbs. Hence Gridhrasi is taken for present clinical study and is approximately correlated to sciatica. Sciatica is a set of symptoms including pain that may be caused by general compression or irritation of one of five nerve roots that give rise to the sciatic nerve. This can also happen by compression or irritation of the sciatic nerve itself. The pain is felt in the lower back, buttock, thigh, leg and foot. Basti karma is praised by all the Acharyas in the management of Gridhrasi to relive from the pain, to improve functional disability and restore functional abilities & controls the condition. The study was conducted with the aim of comparing Ashtakatwaratailamatrabasti with Sahacharatailamatrabasti in the management of Gridhrasi. The clinical trial included 40 patients of Gridhrasi categorized into two groups. 20 patients recieved 75 ml of Sahachara Taila Matrabasti and 20 patients recieved 75 ml of Ashtakatwara Taila Matrabastiafter sthanika abhyanga and swedana for 7 days. Both the groups are having similar effect on reducing the symptoms statistically. Among the subjective and objective parameters B group (Ashtakatwara taila matrabasti) showed better reduction% in Ruk, Sthambha, Toda, right lateral flexion, left lateral flexion and extension of lumbar movement, walking time, VAS, Oswesry disability index.

ABSTRACT

Chronic sciatica is one of the neuropathic pains which assumed to be effective for pregabalin. However, a recent double-blind placebo-controlled trial reported that pregabalin was no more effective than placebo in patients with acute or chronic sciatica. We retrospectively investigated the effect of Kampo medicine including makyoyokukanto extracts combined with powdered cinnamon bark and processed aconite root (makyoyokukanto plus) which were administered to 14 cases with chronic sciatica in the past year. The efficacy of Kampo medicine for lower leg pain relief was assessed using NRS (numerical rating scale). It was effective in 11 of 14 cases (79%, 6 males and 5 females, the mean age : 69), and not effective in 3 of 14 cases (21%, 2 males and 1 female, the mean age : 77). In 7 of 9 cases, who were given pregabalin before prescribing makyoyokukanto plus, makyoyokukanto plus was more effective than pregabalin. Furthermore, we have continued to observe 10 cases for over a year. Two of 10 cases were completely cured even if makyoyokukanto plus was discontinued. In 5 cases we switched from makyoyokukanto plus to yokuininto extracts combined with powdered cinnamon bark and processed aconite root, which was comparable to makyoyokukanto plus in its effect. Makyoyokukanto or yokuininto extracts combined with powdered cinnamon bark and processed aconite root were effective for chronic sciatica.
